R2R – Career Readiness
CAREER READINESS & WORKFORCE DEVELOPMENT SUPPORT
Phase 1: First 30 days of referral
Youth Success Coaches (YSC) engage in monthly Service Planning Meetings (SPM) facilitated by the Social Worker. These meetings include the Juvenile Court Counselor, educators, family, and youth. The purpose of the meeting is to monitor progress and implement strategies for success. The YSC takes this opportunity to understand and discuss career readiness activities, workforce entry, and vocational education.
Phase 2: Three to six months prior to release
Youth Success Coaches begin the development of a Transition Intervention Plan (TIP), which includes career / workforce and vocational education goals after release. The TIP includes information obtained at SPM’s and from meetings with all stakeholders. The YSC utilizes the TIP to begin coordinating for the following:
• Career readiness activities
• Vocational education enrollment
• Professional certification attainment
• Job / workforce placement
Post-TIP discussion, YSCs schedule meetings with identified career/workforce placement to map out a plan for youth’s return. These meetings serve as launch points to discuss youth engagement. YSCs serve as strong advocates for identified career/workforce pathway.
Phase 3: Post Release support up to 12 months
Upon release, the YSC uses the TIP goals to create a First 30 Days plan to ensure that all critical career / workforce components are addressed. The First 30 Days plan includes completion of career interest inventory, identification of job shadowing opportunities, one-on- one job coaching, and volunteering in professional environments. In addition, the YSC assists in coordinating schedules, finding transportation (if needed), and identifying supports to build social competencies.
YSCs monitor goal attainment to ensure youth progress. Engaging with youth to ensure success encompasses the following check-in process:
Stage 1: First 30 Days
• Check Ins: Daily visit (1) or calls (4)
Stage 2: 31 to 60 Days
• Check Ins: 4-times per week
Stage 3: 61 to 120 Days
• Check Ins: 3-times per week
Stage 4: 121 to 365 Days
• Check Ins: 1-time per week
In addition, the YSC engages in monthly Child and Family Team meetings to discuss progress and intervention adjustments. This team includes the family, juvenile court counselors, social workers, and other service providers.
